Full-time Service Coordinator for the Medicaid Waiver Program

Education Requirements: Bachelor's degree in any type of human behavioral services such as human services, social work, psychology; or be a registered nurse. At least two years of professional experience is required in one of the following fields: long-term care, gerontology, rehabilitation, health/disability case management, children with special health care needs, or health/medical.

Flexible scheduling and great benefits offered. Daytime travel required with an agency provided vehicle. Knowledge of computer systems is necessary; however, specialized training will be provided.

RESPONSIBLE TO: CHOICES Program Supervisor

FLSA STATUS: Full-time, Non-Exempt

Purpose of Position: The position will work as a team member within the care programs administered by the Agency. Position will be responsible for a caseload of Nebraska DHHS Medicaid Waiver clients; this will involve assessing the client, developing a comprehensive plan of care, arranging for and developing services to implement the care plan, and necessary follow-up with the client.

EXAMPLES OF WORK PERFORMED:
-Direct provision of services under the Medicaid Waiver Program
-Act upon referrals from the Nebraska DHHS within an established time frame
-Determination of client eligibility for the program according to established guidelines and determining the best pathway to provide and fund services for the client
-Using the standardized assessment tool, assess eligible clients in their environment, i.e., home, nursing facility, or hospital
-Provide clients with information on service options enabling competent adults to make informed decisions as service consumers
-Using professional judgment in determining what is "acceptable risk" and what requires intervention
-Provide a level of advocacy to clients to assure access to services and sufficient knowledge to make informed and appropriate choices
-Develop community resources in cooperation with a resource developer, including efforts to encourage existing long-term care service providers to extend their services in alternative ways to give the client choices in the services they wish to receive
-Assist in service development in areas where services are fragmented or nonexistent
-Develop a plan of care and authorize services that encompass Medicaid funding sources available, including family support, volunteer services, and other program funding sources
-Monitor the delivery and effectiveness of services and client satisfaction
-Comply with all reporting requirements
-Attend training as required for the program
-Compliance with all policies, standards, and best practices of the Agency and the Aged and Disabled Medicaid Waiver program
-Maintain client confidentiality
-Other duties as assigned

MINIMUM QUALIFICATIONS:
-Bachelor's degree in human behavioral services fields such as human services, social work, psychology; OR be a registered nurse
-At least two years of professional experience is required in one of the following fields: long-term care, gerontology, rehabilitation, health/disability case management, children with special health care needs, or health/medical
-Ability to travel to the Agency's seventeen county service areas
-Proficiency with computer skills
-Good verbal and written communication skills
-Valid Nebraska driver's license
-Professional liability insurance
-All Agency employees must pass initial and annual background checks completed on the Central Registry Adult and Child Protective Services, Law Enforcement, Department of Motor Vehicles, Nebraska Data Exchange Network, Sex Offender Registry, Office of Inspector General, and Nurse Registry background checks if applicable
