Job Summary
This position will work closely with both inpatient and outpatient providers of the Advanced Heart Failure service to improve care coordination for the heart failure population.
Skills / Knowledge / Abilities
The ideal candidate for this position will be a self-starter, an independent thinker, as well as have attention to detail.
Does this position have supervisory responsibilities?
No
Preferred Education/Experience
Cardiovascular experience preferred. Ambulatory experience preferred.
