Services Coordinator

The League of Human Dignity seeks full time Services Coordinator to work from our North Platte office with adults with disabilities. Services Coordinators assist people with disabilities to remain in their homes and communities by coordinating an array of services that support the individual in becoming and remaining as independent as possible. We are seeking a talented and highly motivated person who has a sincere desire to assist people.

As a Services Coordinator you will work with participants of the Nebraska Medicaid Waiver Program for Adults with Disability, conduct needs assessment, and in conjunction with Consumers develop a plan of services. You would assist to recruit independent in home providers, negotiate rates, authorize, and monitor services. This is not an exhaustive description of job duties.

In order to be considered for this position, you must have a Baccalaureate degree in relevant area of Human Services, and 2 years of full-time experience in gerontology, rehabilitation, or community health fields are required. Services Coordination will be provided in Consumers’ homes, so travel throughout the west central service area is required. Applicants must also be proficient in the use of computers and various computer software programs. You will be expected to provide your own transportation, and we will reimburse you on a per-mile basis. Qualified individuals with disability are strongly encouraged to apply. EOE
