What You’ll Do

Responsible for the provision of medically related social work services to help residents and clients attain or maintain their highest practicable physical, mental and psychosocial wellbeing.

Responsibilities

· Ensures that all resident rights and responsibilities are protected and enforced, and establishes and maintains a close liaison with residents and families.

· Keeps informed of prevailing Federal and State regulations relating to Long Term Care facilities, best practices in medical social services and disease entities relating to mental and psychosocial matters.

· Participates in admissions, discharges, and evaluation processes for all residents & clients to ensure appropriate assessment and documentation are complete and meet all applicable State and Federal requirements.

o Conducts a comprehensive social history and assessment of resident’s past and current social and emotional needs,

o Collaborates with interdisciplinary team members to ensure that appropriate services are provided to each resident

o Conducts ongoing assessment, monitoring and evaluation of residents progress or lack thereof during their stay

o Identifies and assists in appropriate discharge planning for residents

· Participates in admissions, discharges and evaluation process for residents & clients to ensure appropriate assessment and documentation are complete and meet all applicable State requirements.

o Assists in the development of the Service Plan as relating to Behavioral or Mood related issues; discharge planning and other matters relating to resident’s mental or psychosocial wellbeing.

o Conducts ongoing assessment, monitoring and evaluation of residents’ progress or lack thereof during their stay.

o Assist in collaboration and communication with the resident and/or family members should a higher level of care be required for the resident and discuss options available within the community

o Utilizes Arcadia Family of Companies’ resources to assist residents in being as independent as possible as conditions permit

· Works in close collaboration with the inter-disciplinary team (IDT) to ensure the provision of quality medical social services for residents and clients.

· Assists in planning and managing care plan/service plan conference schedules within mandated time frames. And responsible for contacting residents and/or family members regarding purpose of meetings and invite them to participate as per facility protocols.

· Participates in developing a service plan/care plan to restore residents to optimum social and health adjustment within the resident’s capacity.

· Coordinates with department managers and supervisors to organize and promote educational opportunities for the residents relating to enhancement of residents’ mental and/or psychosocial wellbeing.

· Responsible for the creation/development and facilitation of Resident Council and Family Council meetings. Ensure that minutes of all meetings are taken and documented.

o Schedule monthly meetings in collaboration with selected Resident Council President/chair

o Assist in the development of an appropriate agenda

o Assist in coordination speakers as appropriate and requested by resident council members

o Collaborates with IDT members to provide support to identified resident council needs and requests

o Schedule quarterly Family meetings to provide updates, discuss concerns and/or recommendations of families regarding quality of care/life of residents, develop educational opportunities as requested or identified by families

· Assists with responding to residents, visitors and staff, independently giving information and handling inquiries when possible and as requested by the Administrator or Director of Nursing.

· Ensures that all resident rights and responsibilities are protected and enforced, and establishes and maintains a close liaison with residents and families.

· Monday to Friday, 9:00am - 5:00pm

 

 

Qualifications

Education/Experience

Master’s degree (M.S.W.) in Social Work; or one to two years related experience and/or training; or equivalent combination of education and experience. Experience working with the elderly preferred.

Certificates, Licenses, and Registrations

Current State of Hawaii licensure to practice social work as required by the Hawaii Revised Statues (HRS) 467E.

Computer Skills

Proficiency in using a computer, which may include using such software as Vision or Microsoft Office Suite (Excel, Outlook, PowerPoint, Word) and other applicable software applications.

Physical Demands

While performing the duties of this job, the employee is regularly required to use hands to finger, handle, or feel and talk or hear. The employee frequently is required to sit and reach with hands and arms. The employee is occasionally required to stand and walk. The employee is occasionally required to stoop, kneel, crouch, or crawl.

The employee must frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception, and ability to adjust focus.

Work Environment

While performing their essential functions, employees are occasionally exposed to the following conditions:
